REQUEST FOR PROPOSALS ADVERTISEMENT FOR BIDS
FIVCO Area Development District (FIVCO ADD) & Area Agency on Aging and Independent Living (AAAIL) is requesting proposals for Title IIID Evidence Based Health Promotion and Disease Prevention Services to be provided within the seven senior centers across Boyd, Carter, Elliott, Greenup, and Lawrence counties.
The complete Request for Proposals package can be obtained by sending an e-mail to nicole@fivco.org or visitingwww.fivco.org. The deadline for receipt of submissions is Wednesday, January 15th, 2025, no later than 4:00 p.m. EST.
Submissions will be reviewed by representatives of FIVCO ADD and ranked on the basis of written materials as set forth in the RFP package. FIVCO ADD reserves the right to schedule interviews with any or all respondents or to reject any or all proposals. Kentucky Relay Service for the hearing and speech impaired: 1-800-648-6056.
